7th/12th grade Immunizations

Attn: 7th grade students  Notice of Immunization needed 

The Ohio Health Department requires that ALL students entering the 7th grade for the 2018-2019 school year has been given the following immunizations; 1 dose of Tdap and 1 dose of Meningococcal (serogroup A, C, W, and Y).

Students at Barberton Middle School entering into 7th grade in 2018 will be required to have these immunizations.

Documentation from your child’s healthcare provider must be on file at the school to prevent exclusion. The reason for this early notification is to provide time for you to schedule an appointment with your child’s healthcare provider to receive the required immunizations and/or obtain documentation of compliance in obtaining these vaccinations.

Attn: 12th grade students  Notice of Immunization needed

 

Ohio State Law requires all 12th grade students to receive a meningococcal conjugate vaccine (MCV4). This is a newer requirement.  Students who do not meet these requirements will be excluded from school in the fall of their 12th grade year.

 

The immunization can be obtained through your child’s personal physician, at any walk-in clinic such as the one at Walgreens in Barberton, Akron Health Department, and Axess Pointe Community Health Center located on Robinson Avenue (in the same building as Ritzman’s Pharmacy).

 

You must provide the school with a copy of the shot record. Just drop it off at the Main Office (Office hrs. 7:00 am – 3:00 pm) or mail it to Barberton High School, 555 Barber Rd. to update your records. Those students who are exempted from vaccines must bring in a form signed by the physician indicating the exemption.
